Advancing Implementation

This session will present Canopy's newly updated supportive care and nutrition pathways, including recommended assessments and referral algorithms, for final community input and preparation for site-specific implementation. Key objectives include finalizing the supportive care, nutrition, and electronic patient-reported outcomes (ePRO) pathways and roadmaps, and initiating team discussions to develop implementation strategies. Participants will work to identify potential gaps, formulate one-year aims, and set 90-day goals to guide the successful integration of these pathways into clinical practice.

Ensuring Equitable Access to Timely, Innovative Treatment and Whole-Person Care through Navigation and PROs

This session will highlight the critical role of navigation and patient-reported outcomes (PROs) in providing equitable access to innovative treatments and whole-person care. Featuring member spotlights from the Fred Hutch Supportive Care Program, the session will explore how these approaches enhance patient-centered care. Attendees will gain a clear understanding of how integrating navigation systems and PROs can streamline care delivery, making it more efficient and responsive to patient needs. The discussion will emphasize the synergy between care pathways and navigation, supported by the effective use of PROs.

Advancing Local Implementation of Canopy Supportive Care and Nutrition Pathways

In this session, teams will form to collaboratively formulate a one-year aim and establish at least one actionable 90-day goal to guide the integration of these pathways into clinical practice. This structured team time will provide an opportunity for oncologists and care teams to identify key priorities, address potential barriers, and create a clear roadmap for implementing these patient-centered care pathways within their institutions.

Canopy Member Spotlight Session #3

Teams from several Canopy partners will highlight innovative approaches to cancer diagnostics and treatment. Some emerging topics include: Northwell and UC San Diego to discuss the use of circulating tumor DNA (ctDNA) analysis for treatment decision-making, UC San Francisco and Mt. Sinai to cover the importance of and best practices in germline genetic testing, and Johns Hopkins to share their insights on genomics testing. And complementing these diagnostic advancements, UC San Diego discusses strategies for improving trial enrollment and ensuring timely treatment for patients, underscoring the critical connection between the right tests and the right treatments in enhancing patient outcomes.

This open-space session, facilitated by the Curci Consulting Group, will encourage active participation from Canopy members and stakeholders to collaboratively shape the future of Canopy's strategic initiatives. Attendees will be invited to co-lead, co-create, and co-own projects that drive innovation, sustainability, and impact in cancer care. Through this dynamic and interactive format, participants will work together to define key objectives and ensure a shared commitment to advancing Canopy’s mission in meaningful and lasting ways.
