Thursday, Nov. 7, 2024

8:00 a.m. – 9:00 p.m.

How are we doing now? Where do we want to be in a year? What will it take to get there? Today we dive deep into understanding many of the specifics, from tests to treatments, from care programs to nutrition plans. We'll spend time among the redwoods, crafting individualized 90-day plans and sharing details about patient-reported outcomes and person-centered care. After dinner, we'll cap off the night with more fireside chats.

Friday, Nov. 8, 2024

8:00 a.m. – 5:00 p.m.

Your morning kicks off with a presentation of Canopy’s updated pathways for nutrition and for whole person care, followed by member spotlights from Fred Hutch highlighting successful supportive care and intake/navigation programs. Attendees will work in teams to set actionable goals for the next year, followed by breakout sessions on PROs, pathway dissemination, and Canopy’s potential as a research platform. The day concludes with an open-space session on co-creating a sustainable, high-impact future for Canopy.
