Teams from several Canopy partners will highlight innovative approaches to cancer diagnostics and treatment. Some emerging topics include: Northwell and UC San Diego to discuss the use of circulating tumor DNA (ctDNA) analysis for treatment decision-making, UTSW and Mount Sinai to cover the importance of and best practices in germline genetic testing, and Johns Hopkins to share their insights on genomics testing. And complementing these diagnostic advancements, UC San Diego discusses strategies for improving trial enrollment and ensuring timely treatment for patients, underscoring the critical connection between the right tests and the right treatments in enhancing patient outcomes.

From Hospice to N.E.D. – How Early integration of Palliative Care and Geriatrics can be Lifesaving
Roswell Park Comprehensive Cancer Center will explore the transformative impact of geriatric and “whole-person” assessments on ensuring the integration of supportive and palliative care early in the diagnostic and treatment decision-making process. The Roswell team will present their clinical process and outcomes, and a compelling case study of an 82-year-old man who successfully transitioned from hospice care to achieving no evidence of disease (N.E.D.) through timely interventions. This session will highlight the critical importance of proactive care strategies in enhancing patient outcomes and redefining possibilities in cancer treatment for older adults.

Leveraging Patient-Reported Outcomes for Research and Clinical Care

Principles and Practice of Triple Use
Explore best practices for integrating patient-reported outcomes (PROs) to ensure person-centered care and patient-oriented research. An overview of strategies will be provided for utilizing PROs at the point-of-care, for population-level management, and for improving research outcomes. The session will also emphasize the importance of capturing data in the patient’s own voice, highlighting the differences in information collected by patients versus clinicians. Additionally, representatives from Carevive and UT Southwestern will discuss the critical role PROs will play in shaping the future of clinical trials.

Leveraging PROs and Navigation to Achieve Person-centered care Across the Trajectory
UCLA will present initial insights from its implementation of Carevive and professional navigation systems. The session will focus on how these tools have been utilized to enhance person-centered care, particularly in improving patient intake processes and navigation throughout the care journey. This discussion will highlight the impact of these strategies on delivering more personalized and effective care across the entire patient trajectory.

Harnessing Data in Our Learning Health Network

Accelerating the Path to Triple Use
Explore how to optimize the triple use of data for point-of-care delivery, population-level management, and research within our Learning Health Network. Key highlights include member progress in data standardization and automation, as well as an overview of Canopy's enhanced registry platform, which now boasts updated core elements and new "value-enhancing" data and IT services. We will discuss streamlining our approach by prioritizing data entry efforts through the Canopy MDS (minimum data set) initiative and emphasizing the importance of data automation initiatives within EPIC and other center EMRs, versus the use of third-party platforms. Additionally, we will outline a vision for how novel use of data and enabling IT platforms can facilitate point-of-care access and navigation.
